The Grad College Smart Form App
The Grad College Smart Form app is designed to streamline the submission and management of smart forms within the Graduate College. It provides students, faculty, and staff with a user-friendly interface to complete, validate, and submit required forms electronically. The app supports dynamic form fields, conditional logic, automated routing for approvals, and real-time status tracking. Integration with the Office of Research database ensures secure data storage, efficient workflow processing, and seamless communication between departments.

Smart Forms How To:
- Log In: Students log in to the system using their UTRG credentials.
- Navigate the Home Screen: Proceed to the home screen and ensure you are logged in as a student.
- Select a Form: Choose the form you wish to submit from the available options.
- Complete the Form: Fill out the form with necessary information, ensuring all required fields are completed, especially those not auto populated.
- Track Submission: After submission, monitor the status of your request via the “My Approval” page.
- Receive Confirmation: Once the form is approved, a confirmation email will be sent to your registered email address.
- Make Additional Changes (If needed): If further updates are necessary, such as Change of Committee request, you can submit the relevant form after receiving approval.
- Log In: Use your UTRG credentials to access the system.
- Select Assigned Roles: Choose the role(s) assigned to you within the system.
- Review Submitted Forms: View all submitted forms relevant to your role.
- Provide a Decision: For forms assigned to you, review the details and indicate your decision to approve or deny the form.
- Track Activities: Visit the “Activity” page to view a log of all actions taken regarding the forms, including your decisions.
